GFAP

Glial Fibrillary Acidic Protein (Serum / CSF Astrocyte Biomarker)

Intermediate filament protein of astrocytes released into CSF and blood during reactive astrogliosis; tracks smoldering PIRA progression in MS and amyloid-induced astrocytic reactivity in Alzheimer's.
Target CNS Conditions:Multiple SclerosisAlzheimer's & DementiaFrontotemporal DementiaBroad CNS / Neuroimaging

1. Biological & Technical Mechanism

Glial Fibrillary Acidic Protein (GFAP) is the principal intermediate filament cytoskeleton protein expressed by mature astrocytes in the CNS. Upon neuroinflammation, chronic microglial-astrocytic crosstalk, or blood-brain barrier remodeling, astrocytes upregulate and release soluble GFAP fragments into CSF and blood, quantifiable via ultrasensitive Simoa assays. In Multiple Sclerosis, serum GFAP behaves distinctly from serum Neurofilament Light Chain (NfL): while NfL spikes transiently during acute focal relapses and gadolinium-enhancing lesions, serum GFAP remains stably elevated in patients with chronic active lesions and strongly predicts future Progression Independent of Relapse Activity (PIRA) and gray matter atrophy.

(Japanese Reference):

GFAP(Glial Fibrillary Acidic Protein)は中枢神経系のアストロサイトに特異的に発現する主要中間径フィラメントタンパク質です。

2. Open Science & Clinical Trial Significance

Dual biomarker profiling ('High GFAP / Low NfL' vs. 'High NfL') is standard across modern MS cohorts (Swiss MS Cohort, MSBase) and Alzheimer's plasma panels.

(Japanese Context):
