APOE
Apolipoprotein E (APOE ε2 / ε3 / ε4)
アポリポタンパク質E(APOE遺伝子・ε4アレル)
1. Biological & Technical Mechanism
Apolipoprotein E (encoded by the APOE gene on chromosome 19q13.32) is predominantly synthesized by astrocytes and activated microglia in the central nervous system. It mediates cholesterol and phospholipid transport to neurons via low-density lipoprotein (LDL) receptor family members. Humans carry three major polymorphic alleles—ε2, ε3, and ε4—which differ at amino acid positions 112 and 158. Carrying one copy of APOE ε4 increases Alzheimer's disease risk 3- to 4-fold, while homozygosity (ε4/ε4) increases risk 12- to 15-fold and lowers age of onset.
2. Open Science & Clinical Trial Significance
In open neuroimaging (ADNI, OpenNeuro) and multi-omics cohorts (Agora AMP-AD), APOE genotype is an essential stratification variable. APOE ε4 accelerates amyloid-beta (Aβ) seeding, impairs perivascular clearance, alters microglial lipid metabolism (lipid-droplet-accumulating microglia), and increases susceptibility to Amyloid-Related Imaging Abnormalities (ARIA-E / ARIA-H) during anti-amyloid monoclonal antibody therapy.

Agora Target Validation: APOE (apolipoprotein E) in Alzheimer’s Disease
The protein encoded by this gene is a major apoprotein of the chylomicron. It binds to a specific liver and peripheral cell receptor, and is essential for the normal catabolism of triglyceride-rich lipoprotein constituents. This gene maps to chromosome 19 in a cluster with the related apolipoprotein C1 and C2 genes. Mu...
APOE4/4 Reconceptualized as a Distinct Genetically Determined Form of Alzheimer’s Disease
Analysis of over 10,000 cases suggests that APOE4 homozygosity behaves as a distinct, genetically determined form of Alzheimer's disease with near-complete amyloid penetrance, rather than solely a risk factor.

APOE Genotype and Fluid Biomarkers of Blood-Brain Barrier Integrity: A Systematic Review and Meta-Analysis
