Primary Abstract & Repository Description

Source: medRxiv (Neurology)
BackgroundPoststroke cognitive impairment (PSCI) and poststroke delirium (PSD) remain underexplored. Longitudinal assessments of cognition before PSD onset and at follow-up are scarce, leaving the association between PSD and subsequent global and domain-specific PSCI unclear. Digital assessments using task-derived kinematic parameters may provide complementary insights into cognitive vulnerability and PSD risk. MethodsIn this prospective longitudinal study, patients with acute stroke underwent multidomain tablet-based neuropsychological assessment within the acute phase, before PSD onset, and at 90 days post-stroke. Global PSCI (gPSCI) and domain-specific PSCI (attention and processing speed, executive functions, verbal memory, language, and visuospatial-construction) were assessed using norm-referenced cognitive performance. Cognitive outcomes were compared between patients with and without PSD at both time points. Functional outcome, mortality, and quality of life were assessed at 90 days. Exploratory random forest analyses evaluated the discriminatory ability of acute-phase task-derived kinematic parameters. ResultsAmong 31 patients with stroke, 10 developed PSD, 18 served as controls, and 3 dropped out. At 90 days, patients with PSD had more severe gPSCI than controls (U=19.500, p<0.001), whereas the acute-phase difference was not significant (U=50.500, p=0.057). Domain-specific analyses revealed distinct temporal patterns of cognitive impairment. During the acute phase...

Editorial Critique, Methodological Value & Reusability Guide

Structured via CNS Domain Knowledge Engine

1. Scientific Context & Premise

Primary Premise (medRxiv (Neurology)): "BackgroundPoststroke cognitive impairment (PSCI) and poststroke delirium (PSD) remain underexplored." Within Amyotrophic Lateral Sclerosis (ALS) & Motor Neuron Disease, this work examines medRxiv Clinical, Clinical Neurology, neurology, ALS in relation to plasma/CSF Neurofilament Light Chain (NfL), C9orf72/SOD1/FUS genetics, cryptic exon biomarkers, and ALSFRS-R slopes. Authored by Geiger, L. S., Wortha, F., Klütz, S..

2. Study Architecture & Findings

Study Architecture: Experimental / Clinical CNS Design (primary study cohort) utilizing Clinical Neurology, Preprint Article [DOI: 10.64898/2026.10.21.26366183]. Reported quantitative parameters in the abstract include: 31 patients; p<0.001; p=0.057. Key reported conclusion: "Domain-specific analyses revealed distinct temporal patterns of cognitive impairment."

3. Secondary Analysis & Reusability

Secondary Analysis Applications: (1) Cross-referencing Clinical Neurology, Preprint Article variables (medRxiv Clinical, Clinical Neurology, neurology, ALS) against open repositories in Amyotrophic Lateral Sclerosis (ALS) & Motor Neuron Disease; (2) Benchmarking feature distributions and effect sizes reported in "Cognitive Trajectories of Post-Stroke Delirium: Longitudinal Global and Domain-S"; (3) Informing cohort stratification and endpoint selection.

4. Methodological Caveats & Governance

Preprint Status: This record is an unrefereed preprint; findings should be verified against subsequent peer-reviewed publication. When integrating these Clinical Neurology, Preprint Article findings, account for cohort-specific inclusion criteria (primary study cohort), potential site/instrument batch effects, and repository license terms (cc_by_nc_nd).
