Primary Abstract & Repository Description

Source: medRxiv (Neurology)
BackgroundDespite the clinical importance of acute brain injury (ABI) during extracorporeal membrane oxygenation (ECMO), the mechanism of ECMO-associated ABI remains poorly understood, partly because conventional magnetic resonance imaging (MRI) is incompatible with ECMO devices. Ultra-low-field portable MRI (ULF-pMRI) enables high-quality bedside neuroimaging during ECMO. We aimed to characterize imaging phenotypes of ABI and explore mechanisms underlying lesion distribution across ECMO configurations. MethodsIn this multicenter retrospective observational study, adults receiving ECMO who underwent bedside ULF-pMRI at Johns Hopkins University and University of Texas-Houston from December 2021 to May 2026 were included. ABI was defined as ischemic stroke (IS), intraparenchymal hemorrhage (IPH), subarachnoid hemorrhage (SAH), or subdural hematoma (SDH). Lesions were characterized according to vascular territory, cortical/subcortical lesion versus deep white matter (WM)/basal ganglia (BG) involvement, laterality, lesion burden, and cerebral small-vessel disease (SVD) using Fazekas scale. Associations between ECMO configuration (central venoarterial [VA], peripheral VA, and venovenous [VV]), echocardiographic findings, and ECMO parameters were examined. ResultsAmong 70 patients analyzed (55.9 {+/-} 16.2 years; 54.3% male), 29 (41.4%) had ABI. IS was the predominant type of ABI (82.8%), with most strokes affecting the frontal lobe (48.3%). All the central VA-ECMO patients with ...

Editorial Critique, Methodological Value & Reusability Guide

Structured via CNS Domain Knowledge Engine

1. Scientific Context & Premise

Primary Premise (medRxiv (Neurology)): "BackgroundDespite the clinical importance of acute brain injury (ABI) during extracorporeal membrane oxygenation (ECMO), the mechanism of ECMO-associated ABI remains poorly understood, partly because conventional magnetic resonance imaging (MRI) is incompatible with ECMO devices." Within Central Nervous System (CNS) Neuroscience, this work examines medRxiv Clinical, Clinical Neurology, neurology in relation to multimodal neural oscillations, transcriptomic cell-type taxonomies, structural/functional connectomics, and standardized BIDS/NWB metadata. Authored by Noda, K., Ahmed, Y., Woo, H. Y..

2. Study Architecture & Findings

Study Architecture: Experimental / Clinical CNS Design (primary study cohort) utilizing Clinical Neurology, Preprint Article [DOI: 10.64898/2026.10.21.26366184]. Reported quantitative parameters in the abstract include: 70 patients; 2 years. Key reported conclusion: "All the central VA-ECMO patients with ..."

3. Secondary Analysis & Reusability

Secondary Analysis Applications: (1) Cross-referencing Clinical Neurology, Preprint Article variables (medRxiv Clinical, Clinical Neurology, neurology) against open repositories in Central Nervous System (CNS) Neuroscience; (2) Benchmarking feature distributions and effect sizes reported in "Radiological Phenotypes of Acute Brain Injury During Extracorporeal Membrane Oxy"; (3) Informing cohort stratification and endpoint selection.

4. Methodological Caveats & Governance

Preprint Status: This record is an unrefereed preprint; findings should be verified against subsequent peer-reviewed publication. When integrating these Clinical Neurology, Preprint Article findings, account for cohort-specific inclusion criteria (primary study cohort), potential site/instrument batch effects, and repository license terms (cc_by_nd).
