Advanced MS Neuroimaging Pipeline: Automated Lesion Segmentation, Paramagnetic Rim Lesions (PRLs) & Spinal Cord Atrophy Quantification
End-to-end BIDS neuroimaging workflow for 3D-FLAIR/T1w lesion volumetry, QSM/SWI chronic active rim detection, Central Vein Sign (CVS), and cervical cord cross-sectional area.
With the 2024 revisions to the McDonald Diagnostic Criteria and the shift toward targeting smoldering neurodegeneration, Multiple Sclerosis neuroimaging has evolved far beyond simple T2 lesion counting. Modern computational pipelines integrate isotropic 3D-FLAIR and T1w MP2RAGE with T2* phase / Quantitative Susceptibility Mapping (QSM) and cervical spinal cord MRI. This guide provides a reproducible engineering playbook for automated white/cortical lesion segmentation (FreeSurfer SAMSEG / LST / nnU-Net), Paramagnetic Rim Lesion (PRL) and Central Vein Sign (CVS) quantification, re-baselined brain atrophy modeling, and Spinal Cord Toolbox (SCT) cross-sectional area analysis.
1. From Acute Focal Lesions to Smoldering PRLs, SELs, and the Central Vein Sign
- Co-register isotropic 3D-FLAIR (1 mmÂł) and 3D-T1w before running automated lesion segmentation (FreeSurfer SAMSEG, LST-AI, or MICCAI MSSEG-2 nnU-Net models).
- Perform 'lesion filling' on T1w anatomical volumes prior to FreeSurfer cortical thickness or SIENA brain atrophy estimation to prevent hypointense black holes from skewing gray-white boundary placement.
- Apply a 're-baseline' MRI scan 3–6 months after initiating anti-inflammatory DMTs to separate steroid/anti-edema pseudoatrophy from true neuroaxonal volume loss.
2. Quantifying Cervical Spinal Cord Atrophy & Tract Damage with SCT

- Enforce a minimum lesion size threshold (≥3 mm in long axis or ≥5–10 contiguous isotropic voxels) to prevent false-positive speckle counts.
- Perform white-matter lesion filling before running cortical surface reconstruction or thalamic volume estimation.
- Adjust cervical spinal cord cross-sectional area (C2–C3 CSA) for intracranial volume or spinal canal area, age, and sex.
